Our Case Management & Care Coordination services support your goals by addressing the Social Determinants of Health (SDOH)—things like housing, food, transportation, work, education, technology access, and safety. When these needs are met, therapy is more effective and recovery is more sustainable.

What we do
- Advocate & coordinate: We team up with community programs, medical providers, and agencies on your behalf.
- Connect to resources: Benefits/insurance enrollment, housing and food supports, transportation, childcare, legal aid, employment/education, technology access, and more.
- Reduce barriers to care: Help with paperwork, scheduling, reminders, warm hand-offs, and follow-ups.
- Integrate with therapy: Your case management plan aligns with your treatment plan so your “life goals” and “therapy goals” move together.
Who it’s for
Active NBCC/BCC clients in any level of care (Mental Health, Substance Use, Intensive Outpatient Program, Opioid Treatment Program). Services are individualized, strengths-based, trauma-informed, and culturally responsive.

Counseling Approaches
Care/Case Manager
Primary point of contact; completes needs assessments; coordinates services and follow-ups.
Therapist/Counselor
Leads your treatment plan and integrates SDOH goals into therapy.
Peer Recovery Specialist (as available)
Offers lived-experience support, coaching, and linkage to recovery community.
Nursing & Prescribers
Coordinate medical and medication needs, including MOUD.
Care Coordinator/Front Desk
Scheduling, reminders, forms, and information routing.
Community Partners
Housing, benefits, legal, employment, education, and wellness resources.
How it works (for clients)
- Request support: Ask your therapist or Request an Appointment and select “Case Management.”
- Brief screening: We’ll ask about current priorities across SDOH areas (housing, food, income, transportation, safety, utilities, tech access, etc.).
- Personalized plan: Together we set short-term goals (e.g., submit benefits application, secure transportation) and long-term steps (e.g., job training).
- Resource navigation: We use the FindHelp portal and local partners to match services to your zip code and preferences, then make warm hand-offs when possible.
- Follow-up & problem-solving: We check in, remove obstacles, and adjust the plan as your needs change.
- Transition & closure: When goals are met, we provide a simple transition summary and contacts for ongoing community support.
Confidentiality & consent
Your privacy is protected under HIPAA and, where applicable, 42 CFR Part 2. We’ll request your written consent (Release of Information) before speaking with outside agencies.
